PROMISES, PROMISES: Indian health care's victims

CROW AGENCY, Mont. -Ta'Shon Rain Little Light, a happy little girl who loved to dance and dress up in traditional American Indian clothes, had stopped eating and walking. She complained constantly to her mother that her stomach hurt.
When Stephanie Little Light took her daughter to the Indian Health Service clinic in this wind-swept and remote corner of Montana, they told her the 5-year-old was depressed.
Ta'Shon's pain rapidly worsened and she visited the clinic about 10 more times over several months before her lung collapsed and she was airlifted to a children's hospital in Denver. There she was diagnosed with terminal cancer, confirming the suspicions of family members.
A few weeks later, a charity sent the whole family to Disney World so Ta'Shon could see Cinderella's Castle, her biggest dream. She never got to see the castle, though. She died in her hotel bed soon after the family arrived in Florida.
"Maybe it would have been treatable," says her great-aunt, Ada White, as she stoically recounts the last few months of Ta'Shon's short life. Stephanie Little Light cries as she recalls how she once forced her daughter to walk when she was in pain because the doctors told her it was all in the little girl's head.
Ta'Shon's story is not unique in the Indian Health Service system, which serves almost 2 million American Indians in 35 states.
On some reservations, the oft-quoted refrain is "don't get sick after June," when the federal dollars run out. It's a sick joke, and a sad one, because it's sometimes true, especially on the poorest reservations where residents cannot afford health insurance. Officials say they have about half of what they need to operate, and patients know they must be dying or about to lose a limb to get serious care.
Wealthier tribes can supplement the federal health service budget with their own money. But poorer tribes, often those on the most remote reservations, far away from city hospitals, are stuck with grossly substandard care. The agency itself describes a "rationed health care system."
The sad fact is an old fact, too.
The U.S. has an obligation, based on a 1787 agreement between tribes and the government, to provide American Indians with free health care on reservations. But that promise has not been kept. About one-third more is spent per capita on health care for felons in federal prison, according to 2005 data from the health service.
In Washington, a few lawmakers have tried to bring attention to the broken system as Congress attempts to improve health care for millions of other Americans. But tightening budgets and the relatively small size of the American Indian population have worked against them.
"It is heartbreaking to imagine that our leaders in Washington do not care, so I must believe that they do not know," Joe Garcia, president of the National Congress of American Indians, said in his annual state of Indian nations' address in February.

When it comes to health and disease in Indian country, the statistics are staggering.
American Indians have an infant death rate that is 40 percent higher than the rate for whites. They are twice as likely to die from diabetes, 60 percent more likely to have a stroke, 30 percent more likely to have high blood pressure and 20 percent more likely to have heart disease.
American Indians have disproportionately high death rates from unintentional injuries and suicide, and a high prevalence of risk factors for obesity, substance abuse, sudden infant death syndrome, teenage pregnancy, liver disease and hepatitis.
While campaigning on Indian reservations, presidential candidate Barack Obama cited this statistic: After Haiti, men on the impoverished Pine Ridge and Rosebud Reservations in South Dakota have the lowest life expectancy in the Western Hemisphere.
Those on reservations qualify for Medicare and Medicaid coverage. But a report by the Government Accountability Office last year found that many American Indians have not applied for those programs because of lack of access to the sign-up process; they often live far away or lack computers. The report said that some do not sign up because they believe the government already has a duty to provide them with health care.
The office of minority health at the U.S. Department of Health and Human Services, which oversees the Indian Health Service, notes on its Web site that American Indians "frequently contend with issues that prevent them from receiving quality medical care. These issues include cultural barriers, geographic isolation, inadequate sewage disposal and low income."
Indeed, Indian health clinics often are ill-equipped to deal with such high rates of disease, and poor clinics do not have enough money to focus on preventive care. The main problem is a lack of federal money. American Indian programs are not a priority for Congress, which provided the health service with $3.6 billion this budget year.
Officials at the health service say they can't legally comment on specific cases such as Ta'Shon's. But they say they are doing the best they can with the money they have about 54 cents on the dollar they need.
One of the main problems is that many clinics must "buy" health care from larger medical facilities outside the health service because the clinics are not equipped to handle more serious medical conditions. The money that Congress provides for those contract health care services is rarely sufficient, forcing many clinics to make "life or limb" decisions that leave lower-priority patients out in the cold.
"The picture is much bigger than what the Indian Health Service can do," says Doni Wilder, an official at the agency's headquarters in Rockville, Md., and the former director of the agency's Northwestern region. "Doctors every day in our organization are making decisions about people not getting cataracts removed, gall bladders fixed."
On the Standing Rock Reservation in North Dakota, Indian Health Service staff say they are trying to improve conditions. They point out recent improvements to their clinic, including a new ambulance bay. But in interviews on the reservation, residents were eager to share stories about substandard care.
Rhonda Sandland says she couldn't get help for her advanced frostbite until she threatened to kill herself because of the pain several months after her first appointment. She says she was exposed to temperatures at more than 50 below, and her hands turned purple. She eventually couldn't dress herself, she says, and she visited the clinic over and over again, sometimes in tears.
"They still wouldn't help with the pain so I just told them that I had a plan," she said. "I was going to sleep in my car in the garage."
She says the clinic then decided to remove five of her fingers, but a visiting doctor from Bismarck, N.D., intervened, giving her drugs instead. She says she eventually lost the tops of her fingers and the top layer of skin.
The same clinic failed to diagnose Victor Brave Thunder with congestive heart failure, giving him Tylenol and cough syrup when he told a doctor he was uncomfortable and had not slept for several days. He eventually went to a hospital in Bismarck, which immediately admitted him. But he had permanent damage to his heart, which he attributed to delays in treatment. Brave Thunder, 54, died in April while waiting for a heart transplant.
"You can talk to anyone on the reservation and they all have a story," says Tracey Castaway, whose sister, Marcella Buckley, said she was in $40,000 of debt because of treatment for stomach cancer.
Buckley says she visited the clinic for four years with stomach pains and was given a variety of diagnoses, including the possibility of a tapeworm and stress-related stomachaches. She was eventually told she had Stage 4 cancer that had spread throughout her body.
Ron His Horse is Thunder, chairman of the Standing Rock tribe, says his remote reservation on the border between North Dakota and South Dakota can't attract or maintain doctors who know what they are doing. Instead, he says, "We get old doctors that no one else wants or new doctors who need to be trained."
His Horse is Thunder often travels to Washington to lobby for more money and attention, but he acknowledges that improvements are tough to come by.
"We are not one congruent voting bloc in any one state or area," he said. "So we don't have the political clout."

On another reservation 200 miles north of Standing Rock, Ardel Baker, a member of North Dakota's Three Affiliated Tribes, knows all too well the truth behind the joke about money running out.
Baker went to her local clinic with severe chest pains and was sent by ambulance to a hospital more than an hour away. It wasn't until she got there that she noticed she had a note attached to her, written on U.S. Department of Health and Human Services letterhead.
"Understand that Priority 1 care cannot be paid for at this time due to funding issues," the letter read. "A formal denial letter has been issued."
She lived, but she says she later received a bill for more than $5,000.
"That really epitomizes the conflict that we have," says Robert McSwain, deputy director of the Indian Health Service. "We have to move the patient out, it's an emergency. We need to get them care."
It was too late for Harriet Archambault, according to the chairman of the Senate Indian Affairs Committee, Democratic Sen. Byron Dorgan of North Dakota, who has told her story more than once in the Senate.
Dorgan says Archambault died in 2007 after her medicine for hypertension ran out and she couldn't get an appointment to refill it at the nearest clinic, 18 miles away. She drove to the clinic five times and failed to get an appointment before she died.
Dorgan's swath of the country is the hardest hit in terms of Indian health care. Many reservations there are poor, isolated, devoid of economic development opportunities and subject to long, harsh winters making it harder for the health service to recruit doctors to practice there.
While the agency overall has an 18 percent vacancy rate for doctors, that rate jumps to 38 percent for the region that includes the Dakotas. That region also has a 29 percent vacancy rate for dentists, and officials and patients report there is almost no preventive dental care. Routine procedures such as root canals are rarely seen here. If there's a problem with a tooth, it is simply pulled.
Dorgan has led efforts in Congress to bring attention to the issue. After many years of talking to frustrated patients at home in North Dakota, he says he believes the problems are systemic within the embattled agency: incompetent staffers are transferred instead of fired; there are few staff to handle complaints; and, in some cases, he says, there is a culture of intimidation within field offices charged with overseeing individual clinics.
The senator has also probed waste at the agency.
A 2008 GAO report, along with a follow-up report this year, accused the Indian Health Service of losing almost $20 million in equipment, including vehicles, X-ray and ultrasound equipment and numerous laptops. The agency says some of the items were later found.
Dorgan persuaded Senate Majority Leader Harry Reid, D-Nev., to consider an American Indian health improvement bill last year, and the bill passed in the Senate. It would have directed Congress to provide about $35 billion for health programs over the next 10 years, including better access to health care services, screening and mental health programs. A similar bill died in the House, though, after it became entangled in an abortion dispute.
The growing political clout of some remote reservations may bring some attention to health care woes. Last year's Democratic presidential primary played out in part in the Dakotas and Montana, where both Obama and Democrat Hillary Rodham Clinton became the first presidential candidates to aggressively campaign on American Indian reservations there. Both politicians promised better health care.
Obama's budget for 2010 includes an increase of $454 million, or about 13 percent, over this year. Also, the stimulus bill he signed this year provided for construction and improvements to clinics.

Back in Montana, Ta'Shon's parents are doing what they can to bring awareness to the issue. They have prepared a slideshow with pictures of her brief life; she is seen dressed up in traditional regalia she wore for dance competitions with a bright smile on her face. Family members approached Dorgan at a Senate field hearing on American Indian health care after her death in 2006, hoping to get the little girl's story out.
"She was a gift, so bright and comforting," says Ada White of her niece, whom she calls her granddaughter according to Crow tradition. "I figure she was brought here for a reason."
Nearby, the clinic on the Crow reservation seems mostly empty, aside from the crowded waiting room. The hospital is down several doctors, a shortage that management attributes recruitment difficulties and the remote location.
Diane Wetsit, a clinical coordinator, said she finds it difficult to think about the congressional bailout for Wall Street.
"I have a hard time with that when I walk down the hallway and see what happens here," she says.

Symptoms of Mesothelioma
About 90 percent of people who are diagnosed with malignant mesothelioma have chest pain or shortness of breath as the first symptoms of the disease. Chest pain is caused mainly by the cancer itself, which irritates nerve cells in nearby tissues. Less severe chest discomfort and shortness of breath generally are caused by a collection of fluid between the two layers of the pleura in the chest, not by the cancer itself. This collection of fluid is called a pleural effusion. People with mesothelioma that develops in the abdominal lining can have abdominal pain and swelling caused by an accumulation of fluid in the abdominal cavity. Other possible symptoms include cough, fatigue and unexplained weight loss.
Diagnosis
Your doctor will ask about your symptoms and your medical history before doing a complete physical examination. Tests usually are needed because the more common symptoms of chest pain and shortness of breath may come from many causes. These tests include an electrocardiogram (EKG), a chest or abdominal X-ray, depending on your symptoms and your physical exam. If these tests show any abnormalities of the lungs or pleura, you will need a computed tomography (CT) scan or a magnetic resonance imaging (MRI) scan. These imaging studies allow the physician to determine the size, location and extent of any tumor in the chest or abdomen.
If you have fluid in your chest or abdomen, a thin needle may be used to remove a small sample of the fluid for examination. This procedure, called fine-needle aspiration, also may be used to drain the fluid to relieve symptoms such as chest pain and shortness of breath. Occasionally, mesothelioma can be diagnosed from this fluid sample alone, but usually a tissue sample (biopsy) will have to be taken, too. The tissue sample can be obtained with procedures called a thoracoscopy or video-assisted thoracoscopy (VAT) for a pleural tumor or with a procedure called laparoscopy for an abdominal tumor. In both procedures, a tubelike instrument inserted through a small incision allows the physician to view the tumor and collect a tissue sample. You also may need a procedure called a bronchoscopy or a mediastinoscopy so the doctor can look for masses in the lung airways caused by other tumors or can remove tissue samples from lymph nodes.
Once the diagnosis of malignant mesothelioma has been confirmed, the next step is determining how far the tumor has spread (called staging). Imaging studies, such as computed tomography (CT) and magnetic resonance imaging (MRI), are used to assess the stage of the disease and determine the most appropriate treatment. There are four stages of malignant mesothelioma, with the lowest stage — stage I — having the best prognosis (outlook). In stage I, the tumor is limited to the pleural lining on one side of the chest without any signs of having spread. If the disease returns after being treated successfully, it is called recurrent mesothelioma.
Stage I also is called localized disease, whereas stages II to IV are called advanced disease.
Expected Duration
Once it develops, this type of cancer will continue to grow and spread until it is treated.
Prevention
To reduce your risk of malignant mesothelioma, you should avoid exposure to asbestos. Because there is no safe level of asbestos exposure, any asbestos exposure is too much. In your home, especially if it is an older home, have an expert check for areas of exposed asbestos-containing insulation or other areas of deteriorating asbestos. These areas must be removed professionally or safely sealed off. Workers who routinely deal with asbestos-containing materials should use approved measures to limit their exposure and to keep from bringing asbestos dust home on their clothing.
Treatment
Malignant mesothelioma is a difficult disease to treat. The cancer can spread easily to nearby organs. If the tumor has spread, it is nearly impossible to remove the entire tumor surgically. In addition, it has been difficult to test the effectiveness of different treatments because there are relatively few cases of malignant mesothelioma.
The primary treatment options for malignant mesothelioma are surgery, radiation therapy and chemotherapy.
Surgery
Before any surgery is considered for the treatment of malignant mesothelioma, the patients overall health has to be evaluated. Tests are done to make sure the cancer has not spread to distant sites, and to evaluate how well the patients lungs and heart are functioning. Lung tests look for any signs of lung damage from tobacco or from other diseases, including asthma and chronic obstructive pulmonary disease (COPD). These tests determine how risky surgery would be, especially if a lung needs to be removed.
Surgery for malignant mesothelioma can be aimed at long-term control of the cancer (aggressive surgery) or relief of symptoms (palliative procedures).
Aggressive surgery — A procedure called extrapleural pneumonectomy involves removal of the pleura, the lung, the diaphragm and the pericardium. The intent of this very aggressive, complicated surgery is to remove as much of the tumor as possible. Not all centers will perform this procedure because of its complexity and because it carries a high risk of death within 30 days after surgery. Extrapleural pneumonectomy typically is done only in younger patients who are in good overall health with stage I disease. Patients are evaluated carefully to determine their ability to tolerate the surgery.
Palliative procedures — When malignant mesothelioma is advanced, palliative procedures can be done to relieve or control symptoms such as breathlessness, which are caused by fluid or by the tumor pressing on the lung or other organs. These procedures do not cure the disease.
A procedure called thoracentesis can be used to treat fluid collection (effusion) in pleural mesothelioma. A needle is inserted into the chest to drain the fluid, relieving breathlessness and pain. Talc may be injected into the pleural space to try to stop fluid from accumulating there. This procedure is called talc pleurodesis. Similar procedures are used to relieve fluid collection (ascites) in peritoneal mesothelioma (paracentesis).
A procedure called pleurectomy and decortication is the surgical removal of the pleura. This procedure can be done to reduce pain caused by the tumor or to prevent the fluid from accumulating. For peritoneal mesothelioma, surgery generally is aimed at relieving symptoms.
Radiation Therapy
Because of the location of malignant mesothelioma, it is extremely difficult to deliver high enough doses of radiation to kill the tumor without damaging the surrounding organs. Lower doses of radiation can help to shrink the tumor, but it is unclear whether this helps people to survive longer than if they were not treated.
Using radiation therapy after surgery has not been shown to improve survival. However, because surgery is very unlikely to remove the entire tumor, radiation commonly is done after surgery in the hopes of killing remaining tumor cells. In addition, radiation therapy can be used to relieve the symptoms of mesothelioma, including chest pain.
Chemotherapy
Chemotherapy is the use of medications to treat cancer. Chemotherapy cannot cure mesothelioma. Some chemotherapy drugs have a partial effect in some patients. Combination chemotherapy (using more than one drug at the same time) may be given in an attempt to improve the patients response to the medications. Some combinations have shown some promise, and some new medications are being tried.
Like radiation therapy, chemotherapy may be administered after surgery in an attempt to kill cancer cells that could not be removed during the procedure.
Treatment by Stage
Stage I (localized) mesothelioma — If a patient wants aggressive treatment and is fit enough to have surgery, some centers may do an extrapleural pneumonectomy. Another surgical option is pleurectomy and decortication, which is sometimes done to relieve some of the symptoms of mesothelioma. Both of these procedures may be followed by radiation therapy or chemotherapy.
Stages II, III and IV (advanced) mesothelioma — Pleurectomy and decortication may be done to relieve symptoms in pleural mesothelioma. Other procedures such as thoracentesis may be done to drain fluid (pleural effusions) and prevent them from recurring. Radiation therapy and/or chemotherapy also may be administered for symptom relief.
Recurrent malignant mesothelioma — There is no standard treatment for recurrent mesothelioma. Generally, treatments are considered that were not used the first time the disease was treated.
Clinical Trials and Future Treatments
New treatments for malignant mesothelioma and possible preventive measures, such as a vaccine, are being evaluated in clinical trials, and the future holds some promise. Clinical trials are research studies that evaluate new treatments for safety and effectiveness. There are no guarantees that a new treatment will work, and there are some risks. However, a clinical trial is not undertaken unless the researchers believe the treatment may have some value.
Here are some of the treatments for malignant mesothelioma that are being evaluated:
Combination chemotherapy — Different combinations of chemotherapy drugs have been tried with mixed results.
Intracavitary chemotherapy — Putting chemotherapy drugs directly into the pleural or peritoneal space is being researched because of its advantage over traditional chemotherapy. Because the drug is placed directly into the cavity, much greater doses can be given to patients without causing severe side effects. Some studies have shown this therapy to control effusions and reduce tumor size.
Brachytherapy (intracavitary radiation therapy) — In this treatment, a radioactive substance is placed directly into the pleural or peritoneal space.
Multimodality therapy — Multimodality therapy is any combination of surgery, radiation therapy and chemotherapy.
Gene therapy — In this approach, a virus that has been genetically altered is introduced into the tumor. The virus infects the cancerous cells and makes them vulnerable to anticancer drugs.
Immunotherapy — Treatments that stimulate the bodys immune system to fight cancer cells are called immunotherapy.
When To Call A Professional
Call your doctor promptly for an evaluation if you have any of the symptoms of lung cancer, especially if you have worked in an industry with high exposure to asbestos. If you have shortness of breath or chest pain, you should call your doctor for an evaluation as soon as possible.
Prognosis
Malignant mesothelioma usually is advanced by the time it is diagnosed so the outlook usually is poor. On average, the survival time after diagnosis is about one year. However, several factors affect the prognosis, including the extent of the tumor and the age and health of the patient. Ascorbate deficiency unmasks these underlying genetic defects and leads to an increased plasma concentration of lipids (e.g. cholesterol, triglycerides) and lipoproteins (e.g. LDL, VLDL) as well as to their deposition in the impaired vascular wall. Hypercholesterolemia, LDL-receptor defect.
A multitude of genetic defects lead to an increased synthesis and/or a decreased catabolism of cholesterol or LDL. A well characterized although rare defect is the LDL receptor defect. Ascorbate deficiency unmasks these inherited metabolic defects and leads to an increased plasma concentration of cholesterol-rich lipoproteins, e.g. LDL, and their deposition in the vascular wall. Hypercholesterolemia increases the risk for premature CVD primarily when combined with elevated plasma levels of Lp(a) or triglycerides.
The mechanisms by which ascorbate supplementation prevents the exacerbation of hypercholesterolemia and related CVD include an increased catabolism of cholesterol. In particular, ascorbate is known to stimulate 7-a-hydroxylase, a key enzyme in the conversion of cholesterol to bile acids and to increase the expression of LDL receptors on the cell surface. Moreover, ascorbate is known to inhibit endogenous cholesterol synthesis as well as oxidative modification of LDL.

Hypertriglyceridemia, Type III hyperlipidemia.

A variety of genetic disorders lead to the accumulation of triglycerides in the form of chylomicron remnants, VLDL, and intermediate density lipoproteins (IDL) in plasma. Ascorbate deficiency unmasks these underlying genetic defects and the continued deposition of triglyceride-rich lipoproteins in the vascular wall leads to CVD development. These triglyceride-rich lipoproteins are particularly subject to oxidative modification, cellular lipoprotein uptake, and foam cell formation. In hypertriglyceridemia nonspecific foam-cell formation has been observed in a variety of organs.  Ascorbate-deficient foam cell formation, although a less specific repair mechanism than the extracellular deposition of Lp(a), may have also conferred stability.
Ascorbate supplementation prevents the exacerbation of CVD associated with hypertriglyceridemia, Type III hyperlipidemia, and related disorders by stimulating lipoprotein lipases and thereby enabling a normal catabolism of triglyceride-rich lipoproteins. Ascorbate prevents the oxidative modification of these lipoproteins, their uptake by scavenger cells and foam cell formation. Moreover, we propose here that, analogous to the LDL receptor, ascorbate also increases the expression of the receptors involved in the metabolic clearance of triglyceride-rich lipoproteins, such as the chylomicron remnant receptor.  The degree of build-up of atherosclerotic plaques in patients with lipoprotein disorders is determined by the rate of deposition of lipoproteins and by the rate of the removal of deposited lipids from the vascular wall. It is therefore not surprising that ascorbate is also closely connected with this reverse pathway.
Hypoalphalipoproteinemia.

Hypoalphalipoproteinemia is a frequent lipoprotein disorder characterized by a decreased synthesis of HDL particles. HDL is part of the pathway and is critical for the transport of cholesterol and also other lipids from the body periphery to the liver. In ascorbate deficiency this genetic defect is unmasked, resulting in decreased HDL levels and a decreased reverse transport of lipids from the vascular wall to the liver. This mechanism is highly effective and the genetic disorder hypoalphalipoproteinemia was greatly favored during evolution. With ascorbate supplementation HDL production increases, leading to an increased uptake of lipids deposited in the vascular wall and to a decrease of the atherosclerotic lesion. A look back in evolution underlines the importance of this mechanism. During the winter seasons, with low ascorbate intake, our ancestors became dependent on protecting their vascular wall by the deposition of lipoproteins and other constituents. During spring and summer seasons the ascorbate content in the diet increased significantly and mechanisms were favored that decreased the vascular deposits under the protection of increased ascorbate concentration in the vascular tissue. It is not unreasonable for us to propose that ascorbate can reduce fatty deposits in the vascular wall within a relatively short time. In an earlier clinical study it was shown that 500 mg of dietary ascorbate per day can lead to a reduction of atherosclerotic deposits within 2 to 6 months.
This concept, of course, also explains why heart attacks and strokes occur today with a much higher frequency in winter rather  than during spring and summer, the seasons with increased ascorbate intake.
Other Inherited Metabolic Disorders Associated with CVD.

Beside lipoprotein disorders many other inherited metabolic diseases are associated with CVD. Generally these disorders lead to an increased concentration of plasma constituents that directly or indirectly damage the integrity of the vascular wall. Consequently these diseases lead to peripheral angiopathies as observed in diabetes, homocystinuria, sickle-cell anemia (the first molecular disease described, and many other genetic disorders. Similar to lipoproteins, the deposition of various plasma constituents, as well as proliferative thickening, provided a certain stability for the ascorbate deficient vascular wall. We illustrate this principle for diabetic and homocystinuric angiopathy.

Diabetic Angiopathy.

The pathomechanism in this case involves the structural similarity between glucose and ascorbate and the competition of these two molecules for specific cell surface receptors.  Elevated glucose levels prevent many cellular systems in the human body, including endothelial cells, from optimum ascorbate uptake- Ascorbate deficiency unmasks the underlying genetic disease, aggravates the imbalance between glucose and ascorbate, decreases vascular ascorbate concentration, and thereby triggers diabetic angiopathy.
Ascorbate supplementation prevents diabetic angiopathy by optimizing the ascorbate concentration in the vascular wall and also by lowering insulin requirement.
Homocystinuric angiopathy.

3. After the loss of endogenous ascorbate production, scurvy and fatal blood loss through the scorbutic vascular wall rendered our ancestors in danger of extinction. Under this evolutionary pressure, over millions of years genetic and metabolic, countermeasures were favored that counteract the increased permeability of the vascular wall.
4. The genetic level is characterized by the fact that inherited disorders associated with CVD became the most frequent among all genetic predispositions. Among those predispositions lipid and lipoprotein disorders occur particularly often.
5. The metabolic level is characterized by the direct relation between ascorbate and virtually all risk factors of clinical cardiology today. Ascorbate deficiency leads to vasoconstriction and hemostasis and affects the vascular wall metabolism in favor of atherosclerogenesis.
6. The genetic level can be further characterized. The more, effective and specific, a certain genetic feature counteracted the increasing vascular permeability in scurvy, the more advantageous it became during evolution, and generally, the more frequently this genetic feature occurs today.
7. The deposition of Lp(a) is the most effective, most specific, and therefore most frequent of these mechanisms.   Lp(a) is preferentially deposited at predisposition sites. In chronic ascorbate deficiency the accumulation of Lp(a) leads to the localized development of atherosclerotic plaques and to myocardial infarction and stroke.
8. Another frequent inherited lipoprotein disorder is hypoalphalipoproteinemia. The frequency of this disorder again reflects its usefulness during evolution. The metabolic upregulation of HDL synthesis by ascorbate became an important mechanism to reverse and decrease existing lipid deposits in the vascular wall.
9. The vascular defense mechanisms associated with most genetic disorders are nonspecific. These mechanisms can aggravate the development of atherosclerotic plaques at predisposition sites. Other nonspecific mechanisms lead to peripheral forms of atherosclerosis by causing a thickening of the vascular wall throughout the arterial system. This peripheral form of vascular disease is characteristic for angiopathics associated with Type III hyperlipidemia, diabetes, and many other inherited metabolic diseases.
10. Of particular advantage during evolution, and therefore particularly frequent today, are those genetic features that protect the ascorbate-deficient vascular wall until the end of the reproduction age. By favoring these disorders, nature decided for the lesser of two evils: the death from CVD after the reproduction age rather than death from scurvy at a much earlier age. This also explains the rapid increase of the CVD mortality today from the 4th decade onwards.
11. After the loss of endogenous ascorbate production the genetic mutation rate in our ancestors increased significantly- This was an additional precondition favoring the advantage not only of apo(a) and Lp(a) but also of many other genetic countermeasures associated with CVD.
12. Genetic predispositions are characterized by the rate of ascorbate depiction in a multitude of metabolic reactions specific for the genetic disorder.  The overall rate of ascorbate depletion in an individual is largely determined by the polygenic pattern of disorders. The earlier the ascorbate reserves are depleted without being resupplemented, the earlier CVD develops.
13. The genetic predispositions with the highest probability for early clinical manifestation require the highest amount of ascorbate supplementation in the diet to prevent CVD development. Moreover, ascorbate can prevent blindness and organ failure in diabetic patients, thromboembolism in homocystinuric patients, and many other manifestations of CVD.
Conclusion.

In this paper we present a unified theory of human CVD. This disease is the direct consequence of the inability of man to synthesize ascorbate in combination with insufficient intake of ascorbate in the modem diet. Since ascorbate deficiency is the common cause of human CVD, ascorbate supplementation is the universal treatment for this disease. The available epidemiological and clinical evidence is reasonably convincing. Further clinical confirmation of this theory should lead to the abolition of CVD as a cause of human mortality for the present generation and future generations of mankind.

Hypothyroidism is commonly described as an underactive or sluggish thyroid, but there is more to it than just thyroid activity. Sometimes the problem is thyroid hormone imbalance, primarily underproduction of the thyroid’s main hormone, thyroxine (T4). But T4 also has to be converted into its active form, triiodothyronine (T3), by the liver. It then has to be efficiently utilized by the body. Disruption in any one of these processes can contribute to hypothyroidism symptoms.
Notably, women are far more likely than men to be afflicted with thyroid problems. The great majority of these women suffer from hypothyroidism, which leads to fatigue, weight gain, depression, high cholesterol, low sex drive, feeling cold all the time and many other symptoms. A smaller percentage of women suffer from hyperthyroidism, which is an overactive thyroid.
The connection between thyroid hormone imbalance and menopause is often overlooked. Over 20% of menopausal women in the US are diagnosed with thyroid dysfunction. What causes hypothyroidism in women?
Hypothyroidism is more common in women than in men, probably because hormonal imbalance acts as a trigger for thyroid problems. Women’s bodies have a delicate balance of hormones such as cortisol, DHEA, estrogen and progesterone, which can be upset when your body is under stress and not receiving enough support. Perimenopause, menopause, and pregnancy are times in a woman’s life when hormonal imbalance is particularly common, and are also associated with hypothyroidism.
Dr. John Lee long argued that an excess of estrogen combined with low progesterone — the estrogen dominance typical of early perimenopause — is also a major trigger. Strong synthetic estrogens, such as those in Premarin, may exacerbate estrogen dominance. Birth control pills and patches, medications such as Lupron also contribute to Hormonal imbalances. Dr. Lee also argued that curbing estrogen dominance prevents many problems in perimenopause, including hypothyroidism.
Testing salivary or blood levels of progesterone during the luteal phase of the menstrual cycle can be of great value, so that gentle replacement can be implemented if needed to reduce the negative impact of excess estrogen on the thyroid.
Is it really just a thyroid problem?
The answer is no! I have rarely found a woman who had normal adrenal levels but abnormal Thyroid levels. This is why it is so important to look at the body as a set of interconnected systems, rather than isolating the thyroid as a single entity.
There are so many symptoms of adrenal fatigue that are similar to thyroid disorders that this often leads to confusion in distinguishing between the two. Elevated levels of cortisol impact the thyroid gland on many leves, especially in the conversion of T4 to T3. Women who are in the first stage of adrenal fatigue where they have elevated levels of cortisol will often have problems with this conversion.
Similarly, insulin resistance presents many of the same symptoms as hypothyroidism. Again due to the roles of Adrenal Glands and their regulation of blood sugar levels we of often find a conditional known as insulin resistance. Insulin resistance will also adversely effect both the thyroid and adrenal glands. Its important to note that women with PCOS almost always have a problem with insulin resistance. This is why when we test women we often perform a comprehensive profile which evaluates Adrenal, Thyroid, Insulin sensitivity, Estrogen, Progesterone and Testosterone. Women with hypothyroidism symptoms should also be evaluated for insulin resistance.

When news arrived from the family doctor that Klara had a week to live, Hitler was awestruck. Hitler often viewed himself as a leader, drawing pictures of himself in elementary school leading armies and sitting at a desk dictating orders. Adolf dropped out of high school, not because of his inability to do the work, but because he spent so much time daydreaming about being the leader of a country, in particular, Germany. So after Klaras death in 1907, Hitler decided to pursue his dreams, and become a painter. He moved to Vienna because of this dream, and tried to join The Academy of the Arts in Vienna. The Academy rejected him twice because he had not finished high school or received any other degrees of art in a college setting. Because the Academy was mostly Jewish professors, this is believed to be one of the first accounts of Hitlers Anti-Semitic views towards the Jewish people. (Langer 54-69) At this time he began reading books and articles supporting Anti-Semitism, for reasons mostly unknown. Hitler had Jewish ancestors, but for some reason began to dislike and criticize the Jewish during this time. In Mein Kampf, Hitler describes his transition from opposing Anti-Semitism to supporting it, after seeing an Orthodox Jew:
There were very few Jews in Linz. In the course of centuries the Jews who lived there had become Europeanized in external appearance and were so much like other human beings that I even looked upon them as Germans. The reason why I did not then perceive the absurdity of such an illusion was that the only external mark which I recognized as distinguishing them from us was the practice of their strange religion. As I thought that they were persecuted on account of their faith my aversion to hearing remarks against them grew almost into a feeling of abhorrence. I did not in the least suspect that there could be such a thing as a systematic anti-Semitism.
Once, when passing through the inner City, I suddenly encountered a phenomenon in a long caftan and wearing black side-locks. My first thought was: Is this a Jew? They certainly did not have this appearance in Linz. I carefully watched the man stealthily and cautiously but the longer I gazed at the strange countenance and examined it feature by feature, the more the question shaped itself in my brain: Is this a German? (Hitler 49-95)
After the incident in Vienna, Hitler ran out of money, and began living in a homeless shelter in 1909. In 1910, he had settled into a house for poor working men. Hitler stayed in various homeless shelters and similar areas until 1913 when he joined the Belgian Reserve Regiment. He was a runner, the most dangerous job on the Western Front, and was often exposed to enemy fire. The survival rate for such a job in the military was very low, and Hitler claims later in Mein Kampf that many decisions he made seemed to have come from a higher power. (Hitler 78-92) In one instance in the war, Hitler and his regiment were sitting in a trench eating their dinner. For no particular reason at all, Hitler moved during the dinner to sit with a different group of people, and less than a minute later, the area in which he was sitting was hit by a mortar, and his entire regiment died instantly. It is clear that during and after WWI, Hitler began to feel as if he was alive for a very specific reason. He believed that all of his near death experiences and most importantly that the 15th of October in 1918 had some important significance. (History-Place) On the 15th, Hitler was admitted to a field hospital during the war, being temporary blinded by a mustard gas attack. During this sort of attack, people often experience hysteria and hallucinations. Hitler claims in Mein Kampf that during this time he became convinced that the purpose of his life was to save Germany. (Hitler 49-95) Some scholars, notably Lucy Dawidowicz, argue that an intention to exterminate Europes Jews was fully formed in Hitlers mind at this time, though he probably had not thought through how it could be done. (McDonough 1-19) It is historically known that the mastermind behind the genocide of the European Jews was mostly that of Heinrich Himmler, and not Hitler himself. Hitler remained in the Belgian army after WWI until 1920, but it became clear to him that his destiny was to be the savior of Germany. (Abel 22-59)
In July of 1919, Hitler joined the DAP (German Workers Party) and started to learn more and more about politics. He was particularly interested in how to talk to large crowds and raise morale. Hitler met Dietrich Eckart, an early founder of the party, and Eckart became Hitlers mentor on the issue. Dietrich taught him how to dress and speak, and introduced Hitler to a wide range of people. (Sparticus) Hitler thanked Eckart by paying tribute to him in Mein Kampf. (Hitler Credits) Although Hitler did not become a German citizen until 1932, he lived in Munich and attended plenty of political debates in Beer Hall Putsch, which was the meeting place for Nazi revolutionists in Munich. During a 3 year time period, Hitler and his revolutionists had transformed DAP into the NPP (Nazi Political Party). They were even publishing a weekly paper across Munich and Berlin, and their ideas for governing Germany and bringing it out of a great depression were well received by the German people. (Hit-Org) Its important to remember that after WWI, Germany was forced to pay so much money for what they had done, that the country was on the verge of being completely bankrupt. The Nazi Political Party had ideas for changing Germany and making the country a proud wonderful country again. On the 8th of November in 1923, Hitler and the NPP stormed a public meeting headed by Kahr in the Burgerbraukeller, and requested support of the NPPs new government ideas and the destruction of the Berlin government. The big meeting brought a lot of media attention, Hitler was arrested a day later for the arousal, and sentenced to 4 years in prison for high treason and the attempt to overthrow the government or convince the people that the current government was wrong. Alfred Rosenberg became temporary leader of the NPP, in order to continue to get the Nazi message across to the German people. (Langer 59-101)
Hitler served his prison term from April 1, 1924 to December of that year. He was released because of his immense popularity with the people of Germany. (McDonough 37-91) Today it would be the equivalent of putting a famous star in prison, and during the time he was incarcerated Hitler wrote the immensely popular autobiographical novel Mein Kampf (My struggle, originally entitled Four Years of Struggle against Lies, Stupidity and Cowardice). Mein Kampf at the end of the war was the bestselling book in Germany, because the Fuhrer wanted to make his ideals and messages available to everyone. A copy was given free of charge to every new solider of the German Reich and every newlywed. Hitler strengthened his ideals on the Jewish monster, and ideas of an Aryan race, and everything he had previously supported. It is well known that everything written about in Mein Kampf can be directly related back to his early childhood. (Abel 22-59)
After he was released from prison, everything else became history. The new German political party continued to gain support for its ideas, and was viewed by the people as being the most important thing to ever happen to the country. And, on January 30th of 1933, Adolf Hitler was sworn in as Chancellor of Germany during what some observers later described as a brief and simple ceremony. (McDonough 1-19) He was not known to the German people as a until 1934, when the Nazi party declared that Hitler would be the supreme dictator of the country. Most citizens of the country did not realize that the term would mean absolute leader. Fuhrer in German merely means . After gaining control, Hitler slowly began his plans to turn his country into a super power, and eventually lay in his plans to conquer Europe. (Sparticus)
Adolf Hitler was a very complex individual. He has become known in our society as an entity of evil, when in fact he considered himself to be the new savior of the world. Hitlers decisions during the rest of his career as leader of Germany were believed to be influenced by earlier encounters and things he had come to learn and believe. A large portion of his personality was molded in his childhood and early teenage years, and it can easily be argued to be the primary reason he turned out the person he became. Adolf Hitler can be argued as being among the most influential people in history, because his war alone caused the deaths of more people than almost all other wars combined. A man of extreme intelligence led an entire nation to believe his ideals were correct, and his stage presence often left those listening wailing in tears. A human anomaly, he will be discussed and debated about forever, not only for what he did, but for the person he became and the things he set into action. Adolf Hitler committed suicide in Berlin on April 30th 1945 in a bunker with Eva Braun just days prior to the Red Armys capture of the city.

The music's pretty; I have a string quartet version that didn't make it onto the soundtrack (from a promo CD) that's lovely. It doesn't need Celine Dion or lyrics to work. It sounds better without them.
My Favorite Nominee: Ray of Light, Madonna. I don't have much to say about it, but it works.
My Favorite Single That Year: Flagpole Sitta, Harvey Danger. That song just makes me feel awesome. I love it.

Winner: Sunny Came Home, Shawn Colvin. Music from the nineties has an overwhelming percentage of suck, more than any decade. It's like America went through menopause and could only listen to this kind of sappy pap. I hate this thing, and thanks to the Crap and Crap Lite stations being played where I worked constantly, I heard it way too many fucking times.
My Favorite Nominee: MMMBop, Hanson. It's a default choice; it's the one I think is okay whereas I despise all the others (especially "Where Have All the Cowboys Gone" by Paul Cole, which should be classified a form of abuse).
My Favorite Single That Year: The End Is the Beginning Is the End, Smashing Pumpkins. I love that they used it in the Watchmen trailer.

Winner: Change the World, Eric Clapton. I'm not much of a Clapton fan, really. I did like this song, although it's association with the awful Scientology-promoting John Travolta film Phenomenon counts against it a tad. It's not earth-shaking, but it's a solid, not-unpleasant song.

Winner: Unforgettable, Natalie Cole. The fact that the Grammys honored that hacky, schlocky, sympathy-begging, cloying bit of grave-robbing Natalie Cole did to cash in on honor her father is as sad as it is unsurprising.
My Favorite Nominee: Losing My Religion, R.E.M. It was overplayed, but if you listen to it now, it sounds almost fresh again. It really is just a good song.

Winner: Another Day in Paradise, Phil Collins. Preachy, annoying, and not even the best song from that Phil Collins album. (Actually, I just checked and sadly, it is. I despise "Something Happened on the Way to Heaven," and "I Wish It Would Rain" just sounds like a rip-off of "Wish You Were Here" with Clapton on guitar.)
My Favorite Nominee: Nothing Compares 2 U, Sinead O'Connor. Of the sappy, preachy, sad sack songs that were nominated this year, this is the one that's actually a good song. (Also, "U Can't Touch This" was nominated this year, but come on, man.)
My Favorite Single This Year: Enjoy the Silence, Depeche Mode. Now there's a love song.

Winner: Wind Beneath My Wings, Bette Midler. I hate this song, and my dad pissed me off by playing it at his wedding reception for his mother, which I specifically told him not to do because it was such a fucking cliche. He said he wouldn't; he did. Wow, my grandma must have been one of 10 million special woman so uniquely honored that year. It's the equivalent of buying your dad a tie on Father's Day.
My Favorite Nominee: The End of the Innocence, Don Henley. Chance is right on when he calls it deceptively angry. It adds some world-weariness on top of that, too. Beautiful song.
My Favorite Single That Year: A Little Respect, Erasure. I usually come out on the side of pop, I guess. Although besides the catchiness, I think the lyrics are beautiful. One of my all time favorite lyrics comes from this song: "What religion or reason could drive a man to forsake his lover?"

Winner: Don't Worry, Be Happy, Bobby McFerrin. I always liked this song, but it sure wasn't the best of the year. I think part of it was the novelty of McFerrin doing the whole thing a cappella. Which is admittedly neat.

Winner: Graceland, Paul Simon. I'm not a big fan of this song for whatever reason. It's nice, but it's okay. I wouldn't turn it off if it came on the radio station. Really, I just don't dig Paul Simon's solo work that much.
My Favorite Nominee: Back in the High Life Again, Steve Winwood. Admittedly, mostly because it reminds me of better times. But it's pretty.
My Favorite Single That Year: With or Without You, U2. Grammy nominated the more ubiquitous and much less beautiful "I Still Haven't Found What I'm Looking For," a song I don't like. "With or Without You" is real passion.

Winner: Higher Love, Steve Winwood. Meh. It's okay.
My Favorite Nominee: Sledgehammer, Peter Gabriel. It's a lot of sucky nominees this year, but this is a great song.
My Favorite Single That Year: True Colors, Cyndi Lauper. A beautiful love song, especially for people who don't feel so great about themselves. I guess I like genuine songs about understanding, I would say.

Winner: We Are the World, USA for Africa. Of course. Nothing else was going to win this year. As a song, it's okay. The real fun is trying to pick out all the singers. I mean, you know, it's Really Important, but it's just okay.
My Favorite Nominee: Money for Nothing, Dire Straits. One of their couple of songs I like. One of my favorite guitar solos.
My Favorite Single That Year: Take on Me, a-Ha. Pop perfection in all of its catchy, bubblegum glory.

Winner: What's Love Got to Do with It, Tina Turner. There's genuine force behind it (although I think "Private Dancer" is her best song), real heartbreak.
My Favorite Nominee: Dancing in the Dark, Bruce Springsteen. At his most pop. I love this song.
My Favorite Song That Year: Time After Time, Cyndi Lauper. Gorgeous and simple.

Winner: Beat It, Michael Jackson. Not much of a surprise, I guess. And it's a good song. Eddie Van Halen's guitar solo alone...
My Favorite Nominee: Flashdance... What a Feeling, Irene Cara. All of the nominees this year are pretty good but nothing I feel especially attached to. This is one of those cheesy pop songs I like.
My Favorite Single That Year: Our House, Madness. One of the most perfect songs I've ever loved.

Winner: Rosanna, Toto. It's okay.
My Favorite Nominee: Steppin' Out, Joe Jackson. That one always got me and carried me off.
My Favorite Single That Year: Under Pressure, Queen David Bowie. Everything that's shitty about society in four and a half minutes. "And love dares you to care for the people on the edge of the night."

Winner: Bette Davis Eyes, Kim Carnes. Meh. I don't feel strongly either way.
My Favorite Nominee: (Just Like) Starting Over, John Lennon. What a great song. I can't believe it lost to Kim Carnes... greatness versus... well, nothing worth commenting on. As usual, John Lennon just nails life and relationships with this song.
My Favorite Single This Year: In the Air Tonight, Phil Collins. Collins used to be a man who just knew darkness and how it felt to be depressed and angry.

Winner: Sailing, Christopher Cross. Put me to sleep, why don't ya?

My Favorite Single That Year: Let My Love Open the Door, Pete Townshend. As great a song as he ever wrote for the Who, his best solo work, and one of his most genuinely passionate songs.

Winner: What a Fool Believes, the Doobie Brothers. I'm not a fan of theirs. This is probably the one song of theirs I'd say I liked. Still, Record of the Year? Feh.

Winner: Just the Way You Are, Billy Joel. It is a pretty song, however much Joel claims now that he wrote it accidentally. Is he ever going to stop apologizing for having good commercial instincts? One of his less angry songs, too. I've always liked it.
My Favorite Nominee: Baker Street, Gerry Rafferty. Or as I always used to call it, "That One with the Great Saxophone Part."
My Favorite Single That Year: Who Are You, the Who. My favorite song of theirs, for reasons I can't quite define. But it's a great damn song.

On Saturday, September 27, 2008, the Fullness of Life Foundation hosted the first Arizona screening of Under Our Skin, a documentary film about the untold story of Lyme disease. 
 
Lyme disease is the most common insect-borne disease in America and is one of the fastest growing infectious diseases in the country.  According to the Centers for Disease Control (CDC), while about 20,000 new cases of Lyme disease are reported each year, it is estimated that there are actually over 200,000 new cases per year.  This number is greater than the number of cases of AIDS, West Nile Virus, and the Avian Flu combined.  Cases are so grossly underreported due to a number of factors, including: inaccurate testing, the fear by treating physicians that they will be subject to medical board scrutiny, and physician ignorance regarding how the disease is transmitted and its clinical diagnosis.  According to the CDC, the NIH, and the FDA, Lyme disease should not be ruled out based on tests alone.  Instead, Lyme diagnoses should be based on clinical signs, symptoms, history, exposure risk, course of illness, and testing should play only a supportive role.  Conditions such as Fibromyalgia, Chronic Fatigue Syndrome, multiple sclerosis, ALS, Parkinson’s, and Alzheimer’s, as well as some 350 different diseases, have symptoms similar to Lyme disease.  Known as “the great imitator,” the total number of symptoms associated with Lyme disease can total around 100.  Lyme disease can also carry with it many co-infections that will interfere with its clinical diagnosis.  Prolonged and intense treatment is often needed to rid the body of the disease if it is not caught in its initial stages.  Still, the medical establishment, with input from the insurance industry, has stated that the disease is easily detectable and treatable, and that chronic Lyme disease does not exist.
 
About 250 people attended the Arizona screening of this eye-opening film.  After the showing, Envita’s Chief Medical Officer, Dr. David C. Korn, D.D.S., D.O., M.D.(H), an expert in the diagnosis and treatment of Lyme disease, and Dr. Stephen E. Fry, M.D., a Lyme researcher for over 14 years, were on-hand to answer questions from the audience. 
 
Also in attendance was the film’s senior producer, Kris Newby, who introduced the premier.  Kris is an award-winning screenwriter and science writer, with engineering degrees from Stanford and the University of Utah.  She also has first-hand experience with Lyme disease.  Both she and her husband were infected with it.  But it took them 10 doctors and $60,000 to get diagnosed properly.  Their tests came back positive twice, but they were told by doctors that the chances that they both had it were like winning the lottery.  They were told that they instead were suffering from a couple’s psychosomatic illness.  Kris found out that she and her husband were not the only ones who were told that their debilitating symptoms were “all in their heads.”  Kris was appalled at the way Lyme patients were being treated and misdiagnosed.  After her experiences and struggles with a medical system that refused to acknowledge her illness, she could not just go back to life as usual and pretend as if her experience was an unfortunate but isolated incident.  She had to do something about it.  Together with Andy Abrahams Wilson, the film’s producer, director, and cinematographer, Kris began a four-year, $700,000 project of making the documentary.  Andy’s twin sister had also been diagnosed with Lyme disease and similarly battled physician ignorance and hostility in her quest for proper diagnosis and treatment. 
 
We were blown away by the film.  Kris and Andy traveled to every corner of the United States interviewing Lyme patients and physicians to tell their stories.  The documentary follows seven patients and four physicians as they battle for their lives and livelihoods amidst a health care system that is unwilling and unable to cope with the silent epidemic of Lyme disease.  Interspersed throughout the film are head shots and sound bites from countless other Lyme sufferers.  The film’s greatest testimony to the dire need for proper diagnosis and treatment came from its featured patients.  The difference in the patients who were able to find Lyme-literate physicians to treat them, and their amazing but arduous roads to recovery stood in stark contrast to those patients who were unable to find help.  The film’s worst case scenario of untreated neurological Lyme was a young man named Jared Shea, who is now confined to a wheelchair and is unable to speak after no doctor would diagnose or treat him, instead ascribing his problems to an “unknown etiology.”
 
The film also takes a terrifying look at the politics of the disease, including the 14-member panel that wrote the 2006 Lyme disease treatment guidelines.  This panel was made up of members of the Infectious Disease Society of America (IDSA), a national medical group whose purpose, according to its website, is to improve healthcare in areas related to infectious diseases.  But these guidelines have been widely criticized as inadequate and disastrous for those infected with the disease.  Patient advocates maintain that insurance companies use the guidelines to deny payment for long-term antibiotic use.  According to a recent John-Hopkins study, the IDSA-endorsed two-tiered testing procedure misses 75% of positive Lyme cases. (1)  The guidelines also recommend only two weeks of antibiotic treatment for Lyme disease, yet research has shown that antibiotic treatment for 14 to 21 days results in a 26-50% failure rate, as the stubborn disease-causing bacteria persists much longer in the body.  (2), (3)  The guidelines also negate the existence of chronic Lyme disease, with one panel member instead characterizing it as “more related to the aches and pains of daily living rather than to either Lyme disease or a tick-borne co-infection.”  Out of the 400 scientific articles cited by the panel as support for their guidelines, about half were written by the panel members themselves.  Meanwhile, a large body of contradictory scientific literature was ignored.  The Attorney General of Connecticut, Richard Blumenthal, even conducted an anti-trust investigation of the IDSA, suspecting that the guidelines process was tainted by the panel members’ conflicts of interest.  Out of the 14 member panel, 6 of the panelists or their universities hold patents associated with Lyme disease or its co-infections; 4 received funding from test kit manufacturers for Lyme disease or its co-infections; 4 were paid by the insurance companies to write Lyme policy guidelines or serve as consultants in legal cases; and 9 members or their universities received money from Lyme disease vaccine manufacturers.  Allowing diagnostic and treatment guidelines to be written by panel members who are tied to manufacturers that have a stake in the outcome raises serious and obvious concerns.
 
Equally horrific was the film’s glimpse into the lives of a handful of Lyme-literate physicians.  Most were hauled before their medical licensing boards in a stupendous display of political showmanship and corruption.  Their hearings were nothing more than procedural formalities; the outcomes and the fate of their medical licenses had already been determined as soon as they became known as prominent Lyme physicians.  One doctor, Dr. Joseph Jemsek, M.D., was rejected from serving on the Lyme disease guidelines writing panel, despite the fact that he was one of the leading Lyme-literate private-practice physicians in the southern United States.  Instead, he was charged by the North Carolina Medical Board for improperly diagnosing several patients with Lyme disease and offering controversial intravenous antibiotic treatments.  He was officially sanctioned by the Board, and as the result of having to defend himself against the various accusations against him, ended up bankrupt and had to move his practice to South Carolina.  The film concluded its story on Dr. Jemsek by reporting that he was recently sued by Blue Cross Blue Shield of North Carolina for $100 million for his treatment of Lyme patients.  The majority of the medical complaints against Lyme physicians such as Dr. Jemsek are not from patients, but from insurance companies trying to get rid of doctors that cost them too much money.  The end result of these witch hunts is that patients are losing access to the only physicians who can save their lives.
 
The Arizona screening of Under Our Skin was just one of many that have taken place all across the United States.  Even though Kris and Andy are still looking for a distributor for the film, it has already been shown in over 100 locations simply due to the grassroots efforts of Lyme patients who believe, as Andy does, that “Awareness literally will save lives.”  These patients have taken it upon themselves to host their own screenings for their communities.  One of the most recent screenings took place at the Russell Senate Building on Capitol Hill.  Andy played excerpts of the film for a standing-room only crowd during an historic congressional lunch-hour briefing.  Legislators were invited to a full screening of the film at the Avalon Theater in DC the following day.  Andy then flew to the Camden International Documentary Film Festival, in Rockland, Maine, where the film sold out the 400-seat Strand Theater.  Next, it was on to North Carolina.  Andy and Kris share the task of traveling all over the country to attend these grassroots premier screenings.

advantages in usingthem. By contrast, synthetic preparations (the
prescription drugSynthroid(R),for example) consist solely of T4. They
depend on the body to convertthe T4 to T3 and other metabolites. This
may be fine for some people,but for those whose problem is not a
decline in T4 production but adefect in the conversion of T4 to T3,
taking T4 may not help much.Also, the different thyroid hormones
surely exist in the body for areason, so supplementing with all the
thyroid hormones seems the natural course and makes good sense.
(An example of an animal hormone that is not identical in molecular
structure to its human counterpart is the horse estrogen used in the
drug Premarin(R), which unfortunately is widely prescribed for
estrogen replacement therapy in menopausal women.)

Itis indisputable that elevated cholesterol levels in the blood
areassociated with atherosclerosis and the risk of cardiovascular
disease.However, cholesterol is not the only indicator for
cardiovascular risk.People with elevated levels of theamino acid
homocysteine (Hcy) in their blood are at risk foratherosclerosis and
cardiovascular disease. Some researchers believe that elevated Hcy
levels may be as much or even more of a risk factor than high
cholesterol.

consuming foods and supplements high in the Bvitamins folic acid, B6,

We now also know, however, that low thyroid can lead to high Hcy
levels.In one study, for example, patients who had their thyroid
glandcompletely removed and had stopped thyroid hormone
supplementation wereperiodically tested for blood Hcy and cholesterol,
both of which werefound to be increasing (in a parallel fashion).2When
thyroid supplementation was reinstated, however, both of
thesesubstances were reduced to the patients normal levels within 4
to 6weeks.
Perhaps the best way to combat elevated homocysteine levels,thereby
reducing the risk of heart disease, is to use a combination ofwhole
thyroid extract and the B vitamins (folic acid, B6, and B12)known to
be beneficial for this purpose.

Thyroidhormones have a significant effect on weight. Central to the
manyfunctions of these hormones is the regulation of the bodys
metabolism.They speed up or slow down metabolism as needed, thereby
governing therate at which your body burns calories. Ongoing research
shows that ahealthy thyroid is essential for fat loss.2As our thyroid
function slows down with age, fat storage increases, andwe tend to get
plump. In our youth, it was almost difficult to put onfat. As we age,
